Please Print and make a copy for your records. Include the filled out Rx with your case and your appliance is one step away from being done! Ortho Prime Dental Lab. If you would like us to send actual Rx forms, please make a note and we will send out a stack when your case is delivered. We humbly ask that you provide us with the patients appointment date and time. If the patient is not scheduled, please provide a due date.
Shipping Instructions: Please POUR UP all impressions with a 1-2 inch base. Separate the cast from impression and wrap each cast individually with bubble wrap. Fill out an RX, if you need one please click (link to print RX page). Secure casts and RX in a box and Print out a UPS label. Drop off at any UPS Sto re, UPS drop box, or any Shipping Agent.
